A landmark initiative for the farming community in India , "Yeshasvini Co-operative Farmers Health Care Scheme"(YCFHCS) is a great boon to the Co-operative farmers of Karnataka. The Hon'ble Chief Minister of Karnataka, Sri. S.M. Krishna, inaugurated the scheme on 14th of November 2002, and the scheme was operationalised with effect from 1st June 2003. To state it empathically, Karnataka has become role model state with the introduction of Yeshasvini. Today, any farmer who is a member of a co-operative society in Karnataka can get the necessary treatment and have access to expensive medical procedures by becoming a member of this shceme.

Initiated by Dr. Devi Shetty of Narayana Hrudayalaya, Bangalore, and implemented with suitable modifications by Sri A. Ramaswamy IAS, Principal Secretary to Government of Karnataka, Co-operation Department & band of officers of Co-operation department with M/s Family Health Plan Ltd as the implementing agency, the scheme aims to provide cost effective quality healthcare facilities to the Co-operative farmers spread across the state of Karnataka. The scheme operates under the aegis of the Karnataka State Co-operative Department. Being a self funded scheme the governing body for the implementation, policy decision and financial control lies with the Special Purpose Trust named Yeshasvini Trust with representation from the government of Karnataka, leading doctors and practioners and the implementing agency , M/S. Family Health Plan Limited ( FHPL).

This is the World's largest scheme of Self Funded Healthcare scheme recorded as on date, offering a low priced product for a wide surgical cover, (covering over 1600 defined surgical procedures) to the farmer cooperators and his dependent family members.

This is a contributory scheme wherein the beneficiaries contribute a small amount of money every year to avail any possible surgery during the period. The beneficiaries are offered cashless treatment at the Network of over 135 Hospitals spread across the state of Karnataka.

The Yeshasvini scheme has attracted global attention with two major US bodies, Harvard and the Rockefeller Foundation, planning to study it closely and replicate it elsewhere, especially in African countries. The World Bank too has shown interest in the functioning of this health programme with the intention of finding more pragmatic solutions to low-cost, high-quality healthcare in the developing world.  The International Labour Organisation has showcased the scheme on its website and has also constituted a study of this scheme backed by expert actuaries.
 
    ::: Mechanism :::
 
The scheme is operated by the "Yeshasvini Co-operative Farmers Health Care Trust", incorporated under the India Trust Act. Several prominent state and private individuals act as trustees. The chair of the Trust is the Principal Secretary of the Co-operative department. The Trust has the overall responsibility for the management of the plan.

The plan administration relies on various actors, the government of Karnataka for partial subsidy of benefit, the Karnataka state co-operative department for communication of the plan, co-operative societies enrolling members, co-operative banks to assist in premium collection, Family Health Plan Ltd for the administration of the claims and a network of hospitals to deliver the benefits.
